Easter Creative Art Registration

I understand and acknowledge that participation in events, workshops, and activities organized and/or operated by American Academy of Fine Arts LLC (AAFA) involves inherent and unavoidable risks of injury, harm, illness, or loss. While AAFA takes reasonable precautions to provide proper organization, supervision, and equipment, it is impossible to guarantee absolute safety. I hereby authorize the child/children listed in this registration to participate in AAFA events and activities. On my own behalf and on behalf of the child/children named in this registration, I acknowledge and voluntarily assume all risks associated with participation in AAFA programs. To the fullest extent permitted under the laws of the State of Washington, I hereby waive, release, and discharge American Academy of Fine Arts LLC, including its staff, employees, instructors, volunteers, and agents, from any and all claims, liabilities, damages, or losses arising from or related to participation in AAFA events and activities, including claims alleging negligence. If any portion of this waiver is deemed invalid or unenforceable, the remainder of this agreement shall remain in full force and effect. Medical Authorization In the event of illness or injury requiring medical attention while participating in an AAFA event, I authorize AAFA staff to act according to their best judgment to obtain appropriate medical care for the child/children when I cannot be reached. I understand that it is my responsibility to maintain medical insurance coverage for the child/children participating in AAFA programs and that any medical costs not covered by insurance will be my responsibility. I confirm that the medical information provided is accurate and complete. Health and Communicable Disease Acknowledgment Participation in in-person activities may involve exposure to communicable illnesses, including but not limited to COVID-19, influenza, and other contagious diseases. While AAFA takes reasonable precautions to maintain a safe environment, AAFA cannot guarantee that participants will not be exposed to or become ill. By participating in AAFA events, I voluntarily assume all risks related to exposure to communicable illnesses and agree that AAFA, its staff, employees, and agents shall not be held liable for any illness or related damages resulting from participation in AAFA programs or presence at AAFA facilities. Participants who are experiencing symptoms of illness should remain at home. Conduct and Safety AAFA reserves the right, at its sole discretion, to deny or terminate participation in its programs if a participant’s behavior threatens the safety or well-being of themselves, other participants, parents, or AAFA staff, or otherwise interferes with the operation of the program. AAFA will make reasonable efforts to communicate with the participant and parent/guardian regarding any behavioral concerns. Photo and Media Release I grant permission for American Academy of Fine Arts LLC to take photographs, video, or audio recordings of participants during classes, workshops, or events and to use such media for promotional, marketing, educational, or informational purposes, including publication on websites, social media, printed materials, and other public platforms. I release AAFA and its representatives from any claims or liability arising from the use of such media. Refund Policy A 75% tuition refund will be issued for cancellations made 14 days or more before the event. A 50% tuition refund will be issued for cancellations made 7 days or more before the event. No refunds will be provided for cancellations made within 7 days of the event, although substitutions may be permitted when applicable. Refunds will not be issued for participants who do not attend the event. Cancellation requests must be submitted via email to yanz@aafa.info . A cancellation acknowledgment will be sent within 3 business days, and approved refunds will be processed within 10 business days. Agreement By submitting this registration, I confirm that I have read, understood, and agree to the terms of this Acknowledgement of Risk, Waiver, and Release of Liability for participation in events organized by American Academy of Fine Arts LLC (AAFA).
